The Claims Administrator is now accepting Proof of Claim and Release forms ("Proof of Claim").

You may submit your Proof of Claim using any one of the following methods. Each method is described below, with links to the requirements for each submission. Your submitted Proof of Claim will also include a release of your legal claims against the Defendant and other Released Persons as well as a certification under penalty of perjury that your Proof of Claim is accurate and complete. 

Whichever submission method you choose, please note that supporting documentation of all transactions listed on your Proof of Claim is required.  Include all required and necessary documentation with your submission. In the event that you submit your Proof of Claim online, you must retain confirmation of such filing.

If you fail to execute and submit a timely, properly completed and addressed Proof of Claim, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.

To submit a Proof of Claim by mail, complete and sign the Proof of Claim in its entirety, enclose all required supporting documentation and timely mail it no later than July 30, 2015 to the Claims Administrator at:
